As a benefit-eligible employee, you are automatically enrolled in some benefit programs as of your first day at PRUSA – at no cost to you, such as basic life insurance, accidental death insurance, disability (salary continuation)and the employee assistance program.

You may voluntarily elect to enroll in other benefits, such as medical, dental, vision and supplemental life insurance during specified benefit enrollment periods, where you contribute toward the cost of coverage, as outlined in this guide. Please refer to the employee rate sheet in this guide for your cost to participate in these programs. Eligibility

All US full time employees and part time employees working a minimum of 30 hours a week.

THESE FAMILY MEMBERS:

  • Your spouse or same or opposite sex domestic partner

  • Each child of you or your spouse (or same or opposite sex domestic partner) through the end of the month of each child’s 26th birthday*
